Summary
This study is testing if a team-based clinic can improve the daily life and physical function of people living with a mechanical heart pump (VAD). It will involve 110 patients and their caregivers, comparing new team care to standard visits. The goal is to see if this approach leads to better quality of life, less caregiver stress, and fewer hospital trips.
