Summary
This study is testing whether adding personalized lifestyle coaching from a community health worker to standard remote blood pressure monitoring helps women better manage their heart health after having high blood pressure during pregnancy. It will involve 240 women who had hypertension while pregnant. The goal is to see if the coaching program improves blood pressure control, diet, activity levels, and overall well-being by six months after giving birth, potentially reducing future heart disease risk.
