Summary
This study tests whether a patient navigator — a trained staff member who helps connect people with health services — can improve long-term heart health for those who had high blood pressure disorders during pregnancy. About 60 participants will be randomly assigned to receive either standard care or additional support from a navigator for one year. The goal is to see if this program is practical and acceptable, and whether it helps people attend follow-up appointments and adopt heart-healthy behaviors.
