New clinic aims to keep heart failure patients out of the hospital

Summary

This study tests whether a special clinic can help heart failure patients avoid being readmitted to the hospital after they go home. About 150 adults with a type of heart failure called HFpEF will receive extra education, tools like a blood pressure monitor, and follow-up phone calls. The goal is to see if this support reduces 30-day hospital readmissions and improves quality of life.
