Summary
This study looked at whether having heart failure patients fill out a short, 12-question survey about their symptoms and quality of life at each clinic visit helps doctors provide better care. Over 1,200 adults from a Stanford heart failure clinic took part. Some started the survey right away, while others began a year later. Researchers compared health outcomes and treatment decisions between the two groups to see if routine use of the survey makes a difference.
