Summary
This study is testing a 16-week program that uses a smartphone app, nurse home visits, and family support to help people with advanced heart failure manage their condition after leaving the hospital. It aims to improve patients' quality of life and self-care skills, with the goal of reducing costly and stressful hospital readmissions. The program is being compared to standard heart failure education to see if it's more effective and cost-efficient.
