Summary
This study is testing a 16-week education and support program designed for heart failure patients and their primary caregivers. The program aims to teach them how to work together to better manage symptoms, diet, and medication after a hospital stay. Researchers want to see if this teamwork approach improves patients' quality of life and reduces their need to return to the hospital.
