Summary
This study is testing whether providing home-delivered meals and extra dietary counseling to older veterans after a heart failure hospitalization helps them recover better. It will compare this support to standard dietary advice alone. The main goal is to see if the meals help people stay out of the hospital and improve their quality of life in the first six weeks after going home.
