Summary
This study tested a French national support program (PRADO) for patients after a heart failure hospitalization. It aimed to see if better coordination between doctors and nurses for 2-6 months could reduce costly hospital readmissions and deaths. The research involved 404 patients and looked at both health outcomes and the program's cost-effectiveness.
