Summary
This small pilot study tested whether trained community health workers could help heart failure patients in rural Haiti stick to their care plans after leaving the hospital. The workers visited patients at home and called them to provide education, remind them of appointments, and help manage medications. Researchers wanted to see if this support program was practical, acceptable to patients and workers, and could improve patient health outcomes.
