Summary
This study is testing a special support program for people with a common type of heart failure who are leaving the hospital. The program provides education, a self-care kit, and follow-up phone calls to help patients manage their condition at home. The main goal is to see if this extra support helps patients stay out of the hospital and improves their satisfaction with their care.
