Summary

This study looked at two different types of support for heart failure patients after they leave the hospital. One group got a phone call from a care coordinator, while the other had access to a community paramedic who could visit their home and connect them with a doctor via video. The goal was to see which approach better reduces hospital readmissions and improves quality of life. About 2005 adults with heart failure in New York City took part.
