New program aims to lower heart risks in High-Risk patients

Summary
This study tests a program to help people with high blood pressure and high cholesterol lower their risk of heart disease. About 1,980 adults from Penn Medicine primary care will be assigned to either the program or usual care. The program uses tools and behavior change support to improve blood pressure and cholesterol over 6 months.
