Summary
This study is testing a 6-month remote support program designed to help patients lower their blood pressure and cholesterol. It uses automated messages and a support team to encourage healthy habits and medication use outside of regular doctor visits. The goal is to see if this approach is better than usual care at reducing the risk of future heart attacks and strokes.
