Summary
This study is testing if providing free healthy groceries and coaching helps adults with type 2 diabetes and high blood pressure better manage their health. It focuses on people living in Baltimore neighborhoods with limited access to fresh food. Participants are randomly assigned to receive either basic diet advice or more intensive coaching and grocery support for six months.
