Virtual coaches aim to keep heart patients out of hospital

Summary

This study will test whether adding 12 virtual health coaching sessions over 16 weeks helps people recovering from heart attacks or heart failure. Researchers want to see if coaching improves weight management, reduces stress, increases medication adherence, and lowers hospital readmission rates compared to standard care alone. The study will also ask both patients and their doctors about their experience with the coaching program.
