Summary
This study looks at whether 12 virtual health coaching sessions over 16 weeks can improve the health and well-being of people with heart disease, such as heart failure or a recent heart attack. Researchers will compare those who get coaching plus standard care to those who only get standard care. The goal is to see if coaching helps with weight, stress, medication use, and reduces hospital readmissions and doctor messages.
