Summary

This study is testing if providing free produce boxes, grocery vouchers, and nutrition education can help adults with diabetes or high blood pressure who struggle to afford healthy food. About 400 participants will receive these benefits for 6-12 months. Researchers want to see if this approach improves diet, reduces food insecurity, and leads to better health outcomes like lower blood pressure and blood sugar.
