Summary
This study tests a 24-week church-based program for Black men aged 18 and older who are at risk for heart disease, diabetes, and obesity. Participants attend weekly sessions with exercise and health education, get one-on-one support from a community health worker, and use smart devices to track progress. The goal is to improve blood pressure, cholesterol, and blood sugar levels through lifestyle changes and better access to care.
