Summary
This study is testing a special home visit program for older adults with dementia who were recently hospitalized after a fall. The program, led by an occupational therapist, works with the patient and their caregiver to make the home safer and teach strategies to prevent future falls. Researchers want to see if this program is better than usual care at reducing repeat falls and hospitalizations over one year.
