Remote nurse teams aim to stop dangerous falls in nursing homes

Summary

This study tests three different remote care programs to see which is best at preventing falls and fractures in older adults recovering from a broken bone in skilled nursing facilities. The programs focus on either reducing medications that increase fall risk, improving bone health treatment, or doing both. A nurse consultant and team will work with patients remotely during their stay and after they go home to implement these plans.
