Summary

This study offers a free 3-month diabetes education program at a local faith-based organization for adults with diabetes or prediabetes in Jefferson County, Alabama. Participants attend twice-monthly sessions on diet, activity, and coping, and are paired with a community health worker. Some will use a remote monitoring app to track blood sugar and blood pressure. The goal is to see if this approach improves blood sugar, cholesterol, and weight.
