Summary

This study is for adults with major depression whose first antidepressant didn't work well enough. It compares two next steps: switching to a different standard pill (usual care) versus quickly adding a stronger treatment, esketamine (a nasal spray or IV infusion), to a pill. The goal is to see if the faster, more intense approach controls depression symptoms better and prevents people from developing harder-to-treat depression.
