Summary

This study tested whether a smartphone app (MyREADY Transition CHD) helps 16-17 year olds with congenital heart disease learn to manage their own health as they move from pediatric to adult care. 207 teens used either the app for 18 months or had a single session with a nurse. Researchers measured changes in readiness, heart knowledge, and self-confidence through questionnaires.
