Summary

This study is testing a new support program to help African American and Latinx patients better manage their type 2 diabetes. It provides remote glucose monitoring, a support team, and tailored education to see if this approach improves blood sugar control more than standard care. The goal is to reduce the significant racial gap in diabetes outcomes seen in Chicago.
