Summary
This study tested whether regular meetings between different types of doctors could work well in a hospital setting to coordinate care. It involved 55 patients who have diabetes along with at least one other chronic heart, lung, or kidney condition. The main goal was to see if this meeting system was practical and what patients and doctors thought about it, before planning a larger future trial.
