Summary
This study is testing a digital tool called DIPAR to see if it helps adults with type 2 diabetes get their important health checks done on time. The tool sends reminders to patients' phones when their kidney and blood sugar tests are overdue. Researchers want to know if these reminders lead to more people getting their annual kidney test, which is a key part of managing diabetes.
