Summary
This study tests a digital self-management and peer mentoring program for young adults who survived childhood cancer. The goal is to help them stay on top of their lifelong follow-up care as they move from pediatric to adult health services. The program provides tools and support to improve health knowledge, self-advocacy, and access to care. 300 survivors aged 18-25 will participate.
