Summary
This study is testing a new digital care program where nurses remotely help patients manage their heart health. Participants with high blood pressure or cholesterol will use home devices to track their numbers, and nurses will provide guidance. The goal is to see if this approach helps people better control their risk factors and avoid future heart problems.
