Summary

This study is testing a digital lifestyle platform designed to help patients with COPD, heart disease, fractures, or minor stroke maintain healthy habits after leaving inpatient rehabilitation. About 240 adults will use the platform, which offers personalized tips, educational content, and advice from healthcare professionals. The goal is to see if the platform is easy to use, engaging, and helpful for long-term self-management.
