This study aims to find the most effective way to help adults with uncontrolled type 2 diabetes manage their condition. Researchers are testing whether extra support from a pharmacist, a community health worker, or both, can help people better manage their medications and daily life challenges. The goal is to improve blood sugar control and medication use over six months.
