Summary

This study looks at why some people diagnosed with diabetes in the emergency room don't go to follow-up appointments. Researchers will review medical records, interview patients, and test whether a telehealth visit can help connect them to a regular doctor. The goal is to improve care for 200 adults in New York City and Long Island.
