Summary

This study tests a program that screens hospitalized adults with type 2 diabetes for unmet social needs (like food or housing) and provides support after discharge. About 412 participants will be split into two groups: one gets the new program, the other gets usual care. The goal is to see if addressing social needs reduces emergency visits and readmissions within 90 days.
