Summary
This study tests whether helping people with uncontrolled type 2 diabetes connect with community resources for basic needs like food and housing can improve their health. The program uses community health workers and a new referral system to link 900 patients with local support organizations. Researchers will compare whether this approach helps lower blood sugar levels better than standard diabetes care alone.
