Summary
This study is testing a new support program for young adults with type 1 diabetes as they move from pediatric to adult healthcare. The program uses a team of a nurse, psychologist, and case manager to help with the emotional and practical challenges of this transition. Researchers will compare this new approach to standard care to see if it helps improve blood sugar control and makes the transition smoother.
