Summary
This study tested a new way to help vulnerable patients manage type 2 diabetes within their regular primary care clinic. The clinic offered dedicated diabetes care days with a team of providers who also addressed food insecurity, housing concerns, mental health, and exercise barriers. Researchers trained medical residents in this approach and used continuous glucose monitors to help 65 patients with poorly controlled diabetes.
