New study tests program to keep kidney patients out of hospital

Summary
This study compares a special follow-up care program to usual care for people who had a serious acute kidney injury. The goal is to see if the program helps patients spend more days out of the hospital over 90 days. About 2,145 adults will take part. The program includes extra monitoring and support to prevent further kidney problems.
