Summary

This study tests a self-management program for people with COPD to see which parts work best to reduce hospital stays and improve daily life. Researchers will enroll 448 adults aged 40+ who have had a COPD-related hospitalization in the past year. Participants will use a smartphone or computer to take part, and the goal is to build a program that can be used widely.
