Summary

This study tests a program called CommunityRx-Kidney Health for people with early-stage chronic kidney disease living in rural North Carolina. The program connects patients with medical care, community resources, and self-care tools to improve their health. Researchers will see if it reduces emergency room visits and hospital stays over one year. About 634 adults from local clinics are taking part.
