Summary

This study tests a program called CommunityRx-Kidney Health for people with early-stage chronic kidney disease living in rural North Carolina. The program connects patients with medical care, social services, and self-care resources to help manage their condition. Researchers will see if it reduces emergency visits and hospital stays over 12 months. About 634 adults from local clinics are taking part.
