Summary
This study is testing a new community health worker program to help people in Philadelphia better manage their risk for heart disease. The program combines group education sessions with one-on-one support to help participants develop self-management skills and address social needs like housing or food security. Researchers will compare this enhanced program to standard services to see if it improves participants' ability to care for their health.
