Summary

This pilot study is testing whether adding a trained community health worker to the healthcare team can help children newly diagnosed with type 1 diabetes and their families. The community health worker provides personalized support for one year, helping families navigate challenges like accessing food, healthcare, and medications. The goal is to see if this extra support leads to better diabetes control, fewer hospital visits, and improved quality of life for the family.
