Summary

This study tested whether a comprehensive, year-long community support program could help people with type 2 diabetes manage their condition better than standard medical care alone. About 98 participants in Galveston were assigned to receive either standard care or the new program, which included diabetes education, dietary counseling, physical therapy, and support for mental, financial, and social needs. The main goal was to see if the program improved participants' long-term blood sugar levels and overall health.
