Summary

This study is testing a new team-based care program for adults in primary care who have moderate to severe problems with opioids, stimulants, and/or alcohol. The goal is to see if this coordinated approach helps people reduce their days of substance use and improve their overall health and recovery. The program involves primary care doctors working with other health professionals to provide support and treatment.
