Summary
This study looked at whether having a clinical pharmacist work directly with doctors on hospital rounds can reduce medication problems in patients with kidney disease. 160 adults with reduced kidney function took part. The pharmacist identified and helped fix drug-related issues, especially for medicines that need dose adjustments due to kidney problems.
