Summary

This study tests whether a special tool in the electronic medical record can help doctors lower blood pressure more effectively in patients whose hypertension is not well controlled. About 168 adults with high blood pressure will have their pressure checked 4 weeks after a clinic visit. The goal is to see if the tool leads to better blood pressure numbers compared to usual care.
