Summary
This study tests whether a special app and home monitoring device can help people with advanced chronic kidney disease (stage 5) avoid emergency room visits and hospital stays. About 340 adults will either get usual care or use the VIEWER platform to track their weight and blood pressure at home. The goal is to see if this virtual care approach improves safety and reduces urgent health events.
