Summary

This study aims to find the best way to get people with type 2 diabetes the recommended screening for chronic kidney disease (CKD). Researchers will enroll 750 adults with diabetes who are overdue for kidney tests. They will compare whether sending a home test kit or a standard lab order results in more people completing the screening over 3 months.
