Summary
This study is testing whether an intensive monitoring program helps people with cirrhosis and fluid buildup stay healthier after leaving the hospital. Patients receive a digital scale, educational materials, weekly check-in calls, and early follow-up appointments. Researchers will compare this program to standard care to see if it reduces hospital readmissions, kidney problems, and fluid issues.
