Summary

This study tested whether a community health worker (CHW) program is practical and well-liked by people with dementia and their caregivers after leaving the hospital. Researchers enrolled 34 patients and their caregivers to receive telephone coaching from a CHW. The goal was to see if the program was easy to use and helpful, not to cure the disease.
