Summary

This pilot study tested whether a community health worker (CHW) program could help heart failure patients in rural Haiti stay connected to care. Thirty adult patients hospitalized with heart failure received home visits and support from trained CHWs. The study measured how acceptable, appropriate, and feasible the program was for patients, CHWs, and healthcare staff.
