Summary
This study tests a church-based program where community health workers help adults get screened for colorectal cancer and adopt healthier habits to lower their risk of both cancer and heart disease. It focuses on English-speaking adults aged 45 and older who are not up-to-date with their cancer screening. The goal is to see if this community support model is more effective than usual care at getting people screened and improving their diet and heart health.
