New study aims to make Hospital-to-Home transition safer for cancer patients

Summary
This study tests whether a nurse-led program using online questionnaires can help cancer patients transition safely from hospital to home. About 50 adults with cancer who own a smartphone or tablet will use an app to report their health after discharge. The goal is to see if this approach is practical and improves patient satisfaction and quality of life.
